Are You Qualified For CDS?
Do you think you or your loved one have the qualifications to avail of our consumer-directed services? Determine if your loved one is eligible for CDS. Check if any of these apply.
- Must be 18 years old or older.
- Must receive Medicaid.
- Can self-direct and manage the caregivers they hire.
- Have a medical, or physical disability.
